Responsibilities
Evaluate medical records to obtain necessary medical information to determine patient nutritional care plans. Assess patient nutritional status according to PCMH standards and develops appropriate nutritional care plans. Consult with medical staff and other interdisciplinary team members on appropriate nutritional intervention to improve patient nutritional status or prevent further deterioration of health. Document recommendations in the patient medical record according to department and PCMH policy to communicate with physicians and interdisciplinary team members. Evaluate effects of nutritional intervention on patients' nutritional status to determine need for further intervention or revision of nutrition care plan. Provide nutrition counseling and education to patients and/or family as ordered by the physician or deemed necessary.

Four year degree in Dietetics or Nutrition plus approved Dietetic internship or approved pre-planned experience required. Must be RD or eligible.
