Durham Community Health Network Program
DCHN is seeking a registered Dietitian to join its chronic disease management program for Medicaid recipients in Durham County. This position plays a key role in promoting healthy eating, weight management and medical nutrition therapy for both children and adults. The right candidate for this position will:
Have demonstrated experience working with children and adults who have problems with chronic disease, such as diabetes, obesity and hypertension
Enjoy working with families in a variety of settings, including patient homes, primary care practices and support groups
Possess cultural competency in working with low income and minority populations
Position requirements: This position requires use of a personal vehicle for home visits and other community outreach activities. Current driver license and personal liability insurance is therefore required.
Candidate must be a graduate of an accredited nutrition program, obtained their RD designation, and have current licensure with the North Carolina Board of Dietetics/Nutrition. Candidates with certification from the National Certification Board for Diabetes Educators preferred, but not required. All candidates who are bilingual (English/Spanish) will be given first preference for interview.
